2. Perform a Risk Assessment
Not every issue requires a full CAPA. The impact on product quality, patient safety, and regulatory compliance is evaluated to determine the severity and urgency of the situation.
3. Conduct Root Cause Analysis (RCA)
This is the most critical step. A systematic investigation is conducted to identify the true underlying cause of the problem, not just the immediate symptom. Several tools and techniques can be used, including the 5 Whys and Fishbone (Ishikawa) Diagrams.
A common pitfall is stopping at “human error.” A true RCA digs deeper to find the system failure: Why was the error possible? Was the procedure unclear? Was the equipment poorly designed? Was training inadequate?
4. Develop and Plan Corrective Actions
Once the root cause is known, a plan is developed to correct the specific problem. This could involve equipment repairs, procedure updates, or additional training. Actions must be clear, assigned to specific individuals, and have realistic deadlines.
5. Develop and Plan Preventive Actions
This proactive step seeks to prevent similar issues from occurring elsewhere. The plan should consider where else the same root cause could exist (e.g., other product lines, sites, or suppliers) and implement changes to mitigate that risk.
6. Implement and Verify Effectiveness
Both corrective and preventive actions are executed. Crucially, an effectiveness check must be performed to verify that the actions were successful and did not negatively impact the product or process. This is often a failing point for CAPA systems. “Effectiveness” should be proven by measurable data.
7. Document and Close the CAPA
All steps, including the investigation, decisions, actions, and verification results, are fully documented. This documentation is essential for demonstrating compliance during regulatory inspections. Once all activities are verified, the CAPA is formally closed.
